Welcome. These Terms & Conditions ("Terms") govern your use of this website and its assessment. By using this site, you agree to them. If you don't agree, please don't use the site.
This site is operated by Alpha Men's Health & Testosterone Center, LLC, doing business as Alpha Health & Wellness ("Alpha," "we," "us"). Alpha Inner Circle is a private, application-only membership program of the practice.
The content on this site, including the video, the written material, and the assessment, is provided for general information about the program. It is not medical advice, diagnosis, or treatment, and it is not a substitute for care from a qualified clinician. Using this site, watching the video, or completing the assessment does not make you a patient and does not create a provider-patient relationship. A clinical relationship begins only after you apply, are accepted, and complete enrollment and clinical intake. If you think you are having a medical emergency, call 911.
The assessment on this site screens for fit with the program and explains its structure. It is not a clinical intake or a diagnostic tool. Submitting the assessment or an application is not acceptance into the program. Membership is limited, offered at our discretion, and subject to eligibility requirements, including Florida residency, and to clinical appropriateness determined after acceptance. We may decline any application.
The program is available only to adults who reside in Florida and receive care while located in Florida. Alpha is a private-pay practice: we do not bill commercial insurance, and we do not accept or enroll individuals who are enrolled in Medicare, including Original Medicare and Medicare Advantage. The program is not primary care, emergency care, or hospital care, and it does not replace your other physicians. Members must maintain a primary care provider and appropriate coverage for care outside the program.
Individual health results vary. Nothing on this site promises or guarantees any particular medical outcome, test result, or experience. Descriptions of testing, therapies, and monitoring describe how the program is designed to work, not a guaranteed result for any individual. Whether any treatment, test, or therapy is appropriate for you is an individualized clinical decision made after enrollment.
Certain images on this site are illustrative brand portrayals. The individuals and settings depicted are not Alpha patients or clinicians and do not represent a specific patient experience, outcome, office, or equipment owned by Alpha.
If you submit your contact information, we may contact you about your inquiry by email or phone. If you opt in to text messages, message frequency varies, message and data rates may apply, and you can reply STOP to opt out or HELP for help at any time. Our handling of your information is described in our Privacy Policy on this site. Protected health information, once a clinical relationship exists, is governed by our Notice of Privacy Practices under HIPAA.
The content of this site, including text, structure, program descriptions, graphics, video, and branding, belongs to Alpha and is protected by law. You may view it for personal, non-commercial use. You may not copy, republish, or use it to build a competing offering without our written permission.
Parts of this site, such as the assessment and scheduling, run on third-party platforms. Those services operate under their own terms and privacy practices, and we are not responsible for their independent acts.
To the fullest extent permitted by law, the site is provided as-is, and Alpha is not liable for damages arising from your use of the site itself. Nothing in these Terms limits any right you may have under law that cannot be limited by agreement, and nothing here limits the duties Alpha owes enrolled members under the clinical and membership agreements they sign.
We may update these Terms from time to time. The version posted here, with its effective date, is the one in force. Material changes apply from the date they are posted.
These Terms are governed by the laws of the State of Florida. Any dispute about this website will be handled in the state or federal courts serving Palm Beach County, Florida.
